Taking Control of Your Health
If managing all of your health care needs feels overwhelming, a care manager can help. Care managers are nurses and social workers who can help you better understand and manage your health conditions. They are an advocate, who can coordinate your care with your providers to make it easier for you to achieve your best health.
You, your care manager, and your health care providers can can work together by:
- Assessing your wellness and your personal needs
- Coordinating health care services and supplies
- Building your skills to help you better manage your conditions
- Assisting with referrals to specialty providers
Work with a care manager if you:
- Are being discharged from a hospital stay
- Have a chronic health condition like diabetes, hypertension, or COPD
- Have a high-risk pregnancy
- Want help with advance care planning
Preventive Health
We can provide guidance on services you need like health screenings and immunizations, as well as refer you to our health education classes and health library to help you manage your health and stay as healthy as possible. We may also set you up with a care manager, who can teach you about a new diagnosis, guide you to make the most of your plan, and work with all of your providers.
Transitional Care
Care Managers can help you get appropriate follow-up care after an emergency room (ER) visit, hospitalization or significant life event, including medications or other medical supplies you need to continue your treatment. A care manager’s support, as you transition between treatment settings, can help you better manage your health and reduce the need to go to the hospital.
Complex Care
Partner with a licensed nurse or social worker to help reach your best level of health. Learn more about a new diagnosis or improve your knowledge and skills in managing a chronic health condition. We work with you and your providers to help you understand and take better care of complex health conditions. We help coordinate your care and work one-on-one to make it easier for you to focus on your health.
Behavioral Health
Unsure where to start, or need to talk to someone about your emotional well-being? Receive confidential support from a licensed clinical social worker if you have a mental health or substance abuse concern. A behavioral health clinician can help you:
- Learn more about your mental well-being
- Discuss your treatment needs
- Help you connect to a mental health or substance abuse specialist
- Coordinate your care between your providers
- Offer resources to help you understand and manage your medications
